- Student-athletes must report all athletic injuries/illnesses occurring on the UW-W campus immediately to a certified athletic trainer. All other injuries/illnesses occurring at any off-campus location must be reported prior to resuming athletic participation.
- The Certified Athletic Training staff will make all necessary referrals to our team physicians.
- If a student-athlete chooses to seek medical attention from an outside provider at any time (i.e. family physician, chiropractor, physical therapist, etc.) a written release must be provided to the Certified Athletic Training staff prior to resuming participation. The student-athlete must provide all medical documentation including a diagnosis, ability to resume participation, and rehabilitation needs.
- In the absence of a UW-W Certified Athletic Trainer, the student-athlete should consult with the attending certified athletic trainer regarding any injury/illness.
