Injury Reporting Procedures

In-Season Athletes

  3. If a student-athlete chooses to seek medical attention from an outside provider at any time (i.e. family physician, chiropractor, physical therapist, etc.) a written release must be provided to the Certified Athletic Training staff prior to resuming participation. The student-athlete must provide all medical documentation including a diagnosis, ability to resume participation, and rehabilitation needs.
