Dear Parent or Guardian,
The Athletic Training Staff at the University is pleased that your daughter/son will be participating in Intercollegiate Athletics while at the University of Wisconsin-Whitewater. The purpose of this letter is to describe services offered and to explain what forms are needed prior to any participation.
The Athletic Training Staff consists of four Certified Athletic Trainers (ATC’s) licensed by the State of Wisconsin. The staff provides coverage for all home practices and events. ATC’s duties include prevention of injuries, assessment, treatment, rehabilitation, and consultation to all student-athletes.
UW-Whitewater has a partnership with Mercy Health Care Systems that provides two team physicians (one orthopedic surgeon and one general practitioner with sports medicine training). These physicians visit the athletic training room weekly for evaluations and referrals for student-athletes. Additionally, the physicians provide coverage for many home events. Student-Athletes must be evaluated by an ATC prior to visiting with the physicians. The UW-Whitewater Team physicians have the final say on the participation of an injured/ill student-athlete. Furthermore, LSM Chiropractic provides two team chiropractors who visit twice a week.
Prior to any participation in intercollegiate athletics – including practices or strength and conditioning activities- all necessary paperwork must be completed.
Freshman/Transfers Must complete the following paperwork. Write legible.
- Physical – must be completed on the UW-Whitewater form and must have completed no later than 3 months prior to classes starting. Must be completed by a Licensed Medical Doctor (MD), Doctor of Osteopathy (DO) or Physicians Assistant (PA)
- Health History Questionnaire – Must be completed prior to the physical and signed by the physician completing physical form. All yes answers must be explained.
- Insurance Form – UW-Whitewater Form
- NCAA Insurance Form – Must include copy of front and back of Insurance Card
Returning Student Athletes Must complete the following. Write Legible
UW-Whitewater does not provide insurance for any student-athlete. All student-athletes must provide their own medical insurance for the entire duration of the academic year. The University assumes no legal obligation to pay any such medical expenses due to an injury/illness suffered during athletic participation. The NCAA requires a student-athlete to have a minimum coverage of $75,000.
